(SPRINGFIELD, IL) — Two area nursing facilities have been found in violation of the Illinois Nursing Home Care Act for conditions in which there is a substantial probability that death or serious mental or physical harm will result, or has resulted. The Illinois Department of Public has announced that Aviston Countryside Manor, a 97-bed skilled and intermediate care facility in Aviston, has been cited with an “A” violation and fined $25,000 for failure to safely transfer a resident and prevent a fall, resulting in the resident sustaining a broken leg.  The facility waived its right to a hearing and paid $16,250. In West Frankfort, Frankfort Healthcare and Rehab Center, a 57-bed skilled and intermediate care facility has been cited with an “A” violation and fined $25,000 for failure to address a resident’s adverse reaction to a room change, leading to injection of psychotropic medication and a subsequent fall, resulting in the resident suffering a brain hemorrhage.  The facility waived its right to a hearing and paid $16,250.
